Understanding Our Policy
We ask all patients to review our financial policy carefully. If you have any questions or don’t fully understand something, please contact us now or ask us during your first visit. You will be asked to sign a document acknowledging that you have read and understand the policy, which appears below.
We accept Visa, MasterCard, Discover and American Express cards, Apple Pay, cash or check.
Dr. LeBeau does not participate with any insurance companies nor do we accept assignment of benefits. Payment is expected in full at the time of service. It is your responsibility to file all claims; we will provide you with an Attending Doctor’s Statement to enable you to submit a claim. You will receive direct reimbursement from your dental insurance company. We are always happy to assist you in any way if you have any problems filing your claim or need any further documentation to secure payment from your insurance company. There are times when insurance companies may request that X-rays accompany claims. If that occurs, we will give you a copy of the required X-rays to accompany the claim.

We will file your claims after your dental visit and send statements accordingly unless a pre-authorization has been submitted and your estimated portion is current by your insurance company. A downpayment is required for all procedures over $1000. Please call our office to see if you are eligible as well as to check for updates on the insurance companies with which we participate.
We are aware that occasionally, a situation may arise that may necessitate financial arrangements. Special arrangements may be made with our Financial Coordinator prior to treatment. Payment agreements require approval prior to any services being performed.
